Job Duties

This position has an integral role in quality improvement for Social Work through the discharge
coordination and the mobilization of appropriate resources for patients transitioning to the community. This position assists the Social Work Director by being the primary staff to resolve non-clinical barriers, provide and/or coordinate training for staff, and provide supervision and guidance to social work staff and interns. Provides clinical services as needed to patients in the hospital. Work with staff on barriers to discharge to include identifying, tracking and resolving situations where the patient has been classified as Ready for Discharge (RFD) and there is a delay in discharge. Will build relationships with CSBs to intervene when needed to resolve issues and assist in facilitating discharges. Works with stakeholders including CSBs and other community providers to improve all aspects of the service delivery system for discharge transition. May serve as back up for Social Work Director or other staff as needed.

Minimum Qualifications

-Requires a master’s degree in social work from an accredited school with at least 2 years of clinical social work experience.
-This position may act as a back-up to the Director of Social Work. LCSW is needed to perform some of these job functions.
-Comprehensive knowledge of services provided as well as problems and issues related to state psychiatric hospitals and community mental health systems.
-Knowledgeable regarding NASW Code of Ethics.
-Knowledge of interdisciplinary teams.
-Good understanding of community resources and practices.
-Knowledge of the principles and practices of mental health assessment, psychiatric rehabilitation, and treatment of individuals with serious mental illness.
-Demonstrated ability to use problem solving/ mediation skills.
-Working knowledge of the principles of continuous quality improvement and their application to social work and facility practices.
-Demonstrated ability to manage staff and provide administrative and clinical supervision.
-Knowledge, skills and ability to communicate effectively orally and in writing.
-Ability to perform duties at a fast pace with frequent change.


Additional Considerations

-Knowledge of geriatric mental health is desirable.
-Prior inpatient social work experience is preferred.
-Experience working in/with DBHDS state psychiatric hospitals or CSBs/BHAs is highly desirable.
