The LENOWISCO Health District is looking to fill a Licensed Clinical Social Worker (LCSW) position for its Child Development Clinic in Scott County. This role will involve providing critical support to children and families, assessing developmental needs to ensure the well-being of the community's younger population. The clinic's location is at 190 Beech Street, Gate City, VA.
Purpose of Position: The Clinical Social Worker provides family and client centered services through collaboration to meet their health and well-being. Acts as a resource and team member to deliver quality assistance to all clients. Educates, answers questions, advocates for clients through the appropriate care plan. Assist clients with moving towards independent and serves as a trusted support in situations that require additional assistance.
1. Care Coordination • Through collaboration assesses, plans, implements, coordinates, monitors and evaluates ongoing options and services to meet the health and well‐being of clients • Assesses the medical, financial and psychosocial needs of clients to identify candidates for intensive case management • Uses approved health risk assessment tools to assign appropriate risk level • Develops a comprehensive service plan based on a multidisciplinary assessment for identified potential high risk/complex clients that minimizes these risks 2. Collaboration • Participates as an effective member of the case management/care coordination team to formulate an integrated approach to health care and community services • Acts as a resource to facilitate improved communication, prevention of or resolution of issues related to the client and/or case management in order to achieve continuity of care and efficient utilization of resources • Orients and educates clients/caregivers to the care plan; involves and directs to the most appropriate healthcare resource • Researches appropriateness of and directs clients/caregivers to community agencies or resources for assistance as needed • Serves as an advocate and participates in conferences on behalf of clients to communicate the care plan • Maintains comprehensive collaborative relationships regarding health care needs and issues • Helps clients move toward independence; serves as a trusted support in challenging situations that obstruct positive outcomes • Provides education to community physicians and agencies on issues related to supported program 3. Family Centered Care & Other Duties • Provides family‐centered care; develops and maintains a therapeutic relationship throughout the continuum of care • Consistently demonstrates adaptive communication styles and techniques based on needs; utilizes a culturally competent and non‐judgmental approach to case management • Educates families on rights in the service plan and freedom of choice regarding health care decisions • Assists families in linking with community supports by providing up to date information about resources and support groups specific to identified needs • May be required to perform other duties; may include functions in support of an emergency declaration
This is a restricted position which is solely funded by the Child Development Clinic for a one-year period ending September 30th each year. The availability of funding is scheduled for review annually thereafter. Employment is contingent on the continued availability of grant or non-general funds.
Minimum Qualifications
• Knowledge of principles and practices of professional social work, principles and methods of case management, health education, and in-service education; functions of public and private social service agencies • Skills working with a variety of clients and professionals in a range of complex situations • Additional skills integral in job performance include: assessment skills for identifying problems, goals and interventions during the case management process; interpersonal skills to aid in dealing with difficult clients and in conflict resolution; and problem-solving skills to resolve complex client problems that require the use of considerable judgment and a professional level of knowledge • Ability to communicate effectively, orally and in writing; ability to work independently, organize and prioritize work and work harmoniously with others • Proficiency with the use a computer • Required Licensure: Licensed Clinical Social Worker (LCSW)

• Previous experience in Public Health • Previous experience in an early intervention program • Experience working with diverse cultures • Must be able to provide own transportation for on-duty travel • Knowledge of Part C Local System; knowledge of Part C regulations; • Knowledge of IDEA and/or the Infant & Toddler Connection program; home visitation experience; experience working with parents of young children
