Job Description
The Community Health Worker will have three primary responsibilities. They will promote awareness of the Virginia Cooperative Extension Diabetes Prevention Program (VCE DPP), they will act as a liaison between Medicaid eligible individuals with prediabetes and the VCE DPP Lifestyle Coaches for the enrollment into the program, and will support Medicaid enrollees throughout the program to address social determinants of health needs and remove barriers to retention. To accomplish this the CHW will attend community health events relevant to diabetes across the state with a particular focus in Central Virginia, establish communication and referral systems to the VCE DPP with physician practices in Central Virginia, make contact with individuals on Medicaid referred to the VCE DPP program, and provide CHW services to all enrollees in the VCE DPP program who have Medicaid as their primary insurance. The CHW will also attend regular project team meetings, maintain appropriate records of their activities and outcomes, and assist in preparation of funder reports.
Required Qualifications
Candidates must have certification as a Community Health Worker. Experience with diabetes prevention education, and certification as a lifestyle coach for the National Diabetes Prevention Program lifestyle change program is desirable. Good organizational and communication skills, and ability and/or experience working in coalitions or collaborations. Positions are located in Nottoway, Dinwiddie, Petersburg Counties, Virginia.
Preferred Qualifications
Experience in individual assessment of social determinants of health needs using the PRAPARE tool. Previous experience working with individuals to address their social determinants of health needs. Familiarity with the communities of Central Virginia including Nottoway, Dinwiddie, Amelia, Petersburg, and surrounding counties.
