The Medication Utilization Strategy Pharmacist leads optimization of medication utilization to ensure the safe, clinically effective, and cost-effective medication use across UVA Medical Center. This position will oversee utilization of medications to identify and support development of medication use guidelines, policies, or protocols regarding optimal medication use. This position supports the Pharmacy and Therapeutics Committee and Subcommittees to include formulary management and oversight, facilitation of committees, and development of drug monographs/MUEs/formulary reviews/pharmacoeconomic analysis/and best practice recommendations to the committees.

This position will develop clinical and operational drug shortage management strategies to ensure optimal use of available medications, in collaboration with clinical and operational pharmacy teams. This position will collect, analyze, and disseminate information on medication utilization, and integrate with systems and processes including clinical, safety, prescribing, administration, procurement, and distribution systems to optimize medication use across UVA Medical Center. This position will identify opportunities for stewardship of high cost or high use medications to limit waste, generate revenue, or reduce overall cost of care; executes approved action plans and continuously tracks progress of stewardship initiatives. Additional responsibilities may include strategic partnerships with Service Line leaders, representation on medical center committees, and collaboration on department and inter-professional initiatives.

  • Optimizes medication utilization. -- Leads the department in identification and development of medication utilization optimization initiatives. Identifies high cost, high volume, and/or high risk drugs to monitor appropriate use and develop strategies to optimize use. Develop structured medication use initiatives through collaboration with clinical pharmacy teams, operational teams, pharmacy informatics, departmental leadership, and medical staff. Participates and leads the development of medication use management programs including clinical guidelines, policies, protocols, and other drug use programs to optimize utilization. Perform routine audits to evaluate compliance to and quantify impact of institutional guidelines and initiatives. Prioritizes medications and classes for analysis and review to develop a value-driven formulary. Identifies and completes medication use evaluations (MUEs), which includes assuring appropriate medication use through routine monitoring. Identify, review, and monitor medications with restricted Formulary status for adherence to health-system restrictions. Develop drug shortage management strategies through collaboration with clinical pharmacy coordinators, operational teams, pharmacy informatics, department leadership, and medical and nursing staff.


  • Provides formulary management oversight and support to the UVA Pharmacy and Therapeutics (P&T) Committee and Subcommittees. -- Prepare and/or oversee the preparation of evidence-based drug evaluations and class reviews for P&T committee review. Support and facilitate P&T Committee and Subcommittees including development of drug evaluations/MUEs/formulary reviews/pharmaceoeconomic analysis/and best practice recommendations. Support development and enforcement of the health-system Formulary. Perform ongoing annual Formulary reviews and provide evidence-based recommendations based on optimal medication safety, efficacy, and utilization. Evaluate current Formulary agents using external for quality, safety, and efficacy. Design CPOE templates for new Formulary agents and facilitate changes to CPOE related to formulary management optimization. Review non-formulary utilization reports and identify opportunities to reduce non-formulary utilization in collaboration with clinical pharmacists. E stablish a monitoring system to assess drug utilization/restriction/clinical practice guideline adherence on a routine basis and report results to optimize formulary management. Maintain online formulary. Monitor new drug development/pipeline and proactively anticipates impact to formulary management.


  • Enhances quality of delivery of care. -- Support the Mission, Vision and Values of the Department of Pharmacy Services. Actively participates in the development and implementation of performance/quality improvement and patient safety activities for service area, Pharmacy and Medical Center. Identify potential technical or patient care issues and accept accountability for follow-up and resolution of issues through recommendations and implementation of resolution. Communicates effectively to lead initiatives involving patient care areas, pharmacy operations, procurement, distribution, and clinical teams. Devise methods to quantify impact of new and existing initiatives (eg, formulary management, guidelines, protocols, critical pathways, shortage mitigation, etc) that have been implemented. Report quality concerns, medication errors and adverse drug reactions, as appropriate to internal and external reporting systems (eg., Be Safe, ISMP, etc). Participate in Root Cause Analysis or other quality initiatives (e.g., Be Safe A3) as requested. Represents Pharmacy on institutional/practice area committees and quality support teams. Recommend modifications to improve the CPOE system and reduce medication ordering, preparation, or administration errors. Demonstrates understanding of front-line pharmacy staffing areas and staffs in areas when applicable to support optimization of front-line workflows related to utilization, safety, and quality of care.


  • Supports educational mission. -- Provide didactic and experiential training to pharmacy students and residents. Conform to requirements of Pharmacy Residency Learning System: Develop goals and objectives for each rotation, assign learning activities for each objective, complete and submit summative resident evaluations, electronically to the program director within time limits identified in current Residency Advisory Committee (RAC) policy, complete and submit formative evaluations (snapshots) to the program director within time limits identified in current RAC policy, incorporate the four preceptor roles (direct instruction, modeling, coaching, and facilitation) into all learning experiences (rotations). Consistently attend pharmacy residency seminars and presentations. Provide didactic and/or experiential training to pharmacy students: provide goals of learning expectations to pharmacy students, complete and submit student evaluations to the Pharmacy School in a timely fashion. Provide in-services/lectures to health care professionals in training (medical, nursing, pharmacy). Provide service to institutional/departmental committees related to education and training. Submits and precepts medication use evaluation (MUE) topics and residency project ideas. Serve as pharmacy residency preceptor in any of the following areas: project, MUE, presentations, or as year-long advisor.


  • Engages in professional self-development, scholarly and professional activities.-- Complete required continuing education credits and submit pharmacist license renewal to State Board of Pharmacy on time. Complete competency requirements in a timely manner. Actively participate in goal-setting at the beginning of the performance appraisal period. Establish regular meetings with manager and provide updates regarding progress on goals and initiatives. Enhance professional and technical knowledge/competency by attending class work/other training or journal clubs. Attend professional meetings (funding permitted); identify one meeting/year to attend and provide formal feedback to pharmacy staff regarding key items from the meeting. Attain/ maintain professional certifications. Participate in multidisciplinary and departmental research initiatives. Submit publications to newsletters, bulletins and/or professional journals. Provide in-services/lectures to health care professionals. Demonstrate active membership and service (committee/council/commission/task force member or elected/appointed officer) in local, state or national pharmacy organizations.


Position Compensation Range: $50.43 - $80.69 Hourly

MINIMUM REQUIREMENTS

EDUCATION: Doctor of Pharmacy required.

EXPERIENCE: PGY-1 Required, PGY-2 Preferred (Clinical Specialty, HSPA, Managed Care, Informatics, Medication Use Safety, Drug Information, Medication Systems, Medication Management, or other) or 3 years hospital pharmacy experience.

LICENSURE: Commonwealth of Virginia State Board of Pharmacy License.
