The UVA Children’s Hospital Division of Pediatric Hematology/Oncology seeks an RN Care Coordinator. The successful candidate will join a dedicated and caring multidisciplinary team consisting of MDs, NPs, a Licensed Clinical Social Work, a Psychologist, Clinical Research Coordinators, an Education Consultant and other professional staff to enable the best possible service to these complex patients and their families. The role involves management and coordination across the entire care continuum in partnership with our providers and support staff. This will include insuring proper scheduling of outpatient visits, hospital admissions, procedures, imaging studies and other elements of patient care, as well as securing prior authorizations for treatment and coordinating care with home health providers. Participation in pre-clinic care coordination conferences will be required daily to insure a proactive approach to patient care needs. Bi-directional communication with patients and families, in-person, by telephone and via the electronic medical record is a critical component of this position.
A dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team. This registered nurse utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care.

Position Compensation Range: $65,249.60 - $107,660.80 Annual
MINIMUM REQUIREMENTS
Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire. Experience: 1 year of relevant experience. Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.
PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
