Within the inpatient setting to assess and address patient and family needs related to effective and efficient movement through the hospital system and beyond. Perform initial assessment on all patients and begin discharge process from day one of patient’s stay. Complete level of care assessment to ensure patients are receiving the appropriate level of care, and to ensure that the patient transitions through their stay without barriers or delays in care delivery. Maintain expert knowledge of Interqual® Level of Care criteria. Work closely with the Admitting Physicians to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care. Coordinate discharges home and facilitate transfers to external placement facilities

Position Compensation Range: $67,849.60 - $108,555.20 Annual
MINIMUM REQUIREMENTS
Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire. Experience: 3 years Relevant Experience Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. Accredited Case Manager (ACM) Certification within 3 years of hire.
PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
