Advanced Practice Provider (NP/PA/APP) - UVA Health Mobile Care

The Advanced Practice Provider (APP) provides comprehensive primary care services to adult and pediatric patients through the University of Virginia Health System’s innovative Mobile Care Unit and associated population health programs. This position supports UVA Health’s mission to improve access to high-quality care and advance health equity through innovative, community-based care delivery models that unlock premiere care resources to the broader population, especially those in historically underserved areas.communities.

The APP will practice in collaboration with an interdisciplinary care team including nurses, program-affiliated paramedics, care coordinators, physicians, social workers, APP colleagues, and others. In addition to direct mobile care, the successful candidate will support our Interactive Home Monitoring (IHM) and primary care bridging programs, supporting continuity of care for patients across the care continuum. The position is administratively affiliated with the Department of Population Health and professionally supported by the Center for Advanced Practice, UVA Health’s professional home for advanced practice providers.

Given the unique and highly-independent nature of this opportunity, autonomous practice licensure is required.

Hours are Monday-Friday with an expectation of 32 patient-facing clinical hours weekly. A cover letter included in your application packet is required.

Preferred Qualifications:

• 3+ years of experience in adult primary care, community health, or mobile health settings
• Experience caring for underserved or high-risk patient populations.
• Willingness to travel to various locations within the community served by the mobile clinic van.
• Familiarity with population health, care management, or transitional care models.
• Experience with telehealth, remote patient monitoring, or virtual care platforms.
• Strong interpersonal, communication, and teamwork skills.
• Experience practicing within an academic medical center or integrated health system

Advanced Practice Provider Scope of Practice:

Clinical

• Independently provide comprehensive evaluation, diagnosis, treatment, and management of adult and pediatric primary care patients in mobile and community-based settings.
• Manage acute and chronic medical conditions using evidence-based practice guidelines.
• Support Interactive Home Monitoring (IHM) through remote clinical assessment, monitoring, and escalation of care as appropriate.
• Participate in primary care bridging initiatives to facilitate timely follow-up and care continuity following emergency department or inpatient encounters.
• Collaborate with program-affiliated nurses, paramedics, physicians, care coordinators, social workers, pharmacists, and other partners to address clinical and psychosocial needs.
• Document clinical encounters accurately and efficiently in the electronic health record in accordance with regulatory, quality, and billing requirements.

Administrative

• Support operational workflows related to mobile care delivery, including care coordination and clinical documentation.
• Participate in quality improvement, performance measurement, and program evaluation activities.
• Assist with data collection and reporting related to access, outcomes, and patient experience.
• Comply with all UVA Health policies, procedures, and regulatory requirements.
• Participate in departmental meetings, committees, and institutional initiatives as assigned.

Leadership

• Serve as a collaborative member of the Mobile Care Unit team, contributing to a culture of high-quality, patient-centered care and driving program expansion.
• Provide informal guidance and support to APP colleagues, learners, or new team members as appropriate.
• Participate in Center for Advanced Practice activities that support APP professional practice and development.
• Identify opportunities to enhance care delivery, team efficiency, and patient engagement across mobile and population health programs.

Professional

• Maintain active national certification and Virginia licensure consistent with scope of practice.
• Engage in continuing education and professional development to maintain clinical competence.
• Demonstrate professionalism, accountability, and commitment to UVA Health values.
• Participate in scholarly, educational, or quality improvement activities as aligned with departmental practice and expectations.

MINIMUM REQUIREMENTS

Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree.

Experience: Department and unit specific.

Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease.

The pay range for this role is $116,126.00 - $171,163.20 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
