Job Duties
Directly responsible to Lead Treatment Team Coordinator to coordinate the development of comprehensive and individualized treatment plans that are inclusive of mental health, substance use and medical/nursing care needs. As a member of the Treatment Team, the Treatment Team Coordinator monitors and ensures the accurate and timely completion of treatment care plans and all required documentation relevant to treatments and services that are consistent with the individually planned, sound, and therapeutic interventions that conform to current acceptable professional practice.
Minimum Qualifications
Experience and knowledge of psychiatric treatment pertaining to the adult and geriatric age groups.
Experience working on an interdisciplinary team and experience with treatment care plan development.
Experience utilizing clinical documentation standards and interpreting other relevant resources, to include DSM-V, Physician’s Drug Reference Book, TJC and CMS standards of care, etc.
Experience interpreting documentation in medical records, other reports, and requirements as they relate to clinical practice and make recommendations for improvement.
Experience with evidence-based psychosocial treatments for common mental disorders and the ability to utilize stages of change, motivational interviewing techniques, and shared decision-making tools to promote the development of person-centered treatment plans.
Experience composing a Master Treatment Plan inclusive of goals, objectives and interventions specific to each patient’s individualized needs.
Experience working with psychiatric disorders, substance abuse disorders, and physical conditions.
Experience developing and evaluating non-pharmacological interventions to address biopsychosocial needs of patients with varying degrees of acuity and complexity.
Extensive experience using Microsoft Office programs, including Word, Excel, and Outlook and Electronic Health Records.
Additional Considerations
Extensive experience working in an inpatient psychiatric hospital, preferred.
