The Chesapeake Health Department is seeking a resilient Public Health Nurse with a drive to promote health, preventing disease and support the community. The Long-Term Support Services (LTSS) is a Department of Medical Assistance Services (DMAS) mandated program. The Registered Nurse (RN) / Public Health Nurse (PHN) works in partnership with a Social Worker (SW), from the local Department of Social Services (DSS), to conduct joint home visits for adults that are imminent risk for nursing facility placement. Both the RN and SW travels throughout the City of Chesapeake to complete each screening. The RN / PHN will also be responsible to provide screenings for pediatrics, this visit is done without a SW. The PHN will perform other duties to support the district; other services include immunization, emergency preparedness and response (EP&R) and communicable disease (CD) responses. The ideal candidate will be experienced in performing nursing assessments across the lifespan in both clinical and community settings. This position provides additional experiences with case management, resource referrals, computerized charting, EMR, and proficient use of Microsoft applications as necessary. Previous home care visiting experience is a plus.
Personal vehicle travel is required for this position. This position is for 40 hours per week Monday – Friday 8:15 am – 5 pm, including 45 minutes for lunch.
Please note in the event of a local public health emergency and / or an emergency declaration by the Governor, this position may be required to assume other duties for the districts’ emergency preparedness and response (EP&R) activities. During declared emergencies, this will require an adjustment to work hours, location, job duties as well as participation in trainings to meet public health business needs. Additional potential work-schedule variations including participation in community outreach events and other EP&R assignments, such as communicable disease and epidemiological investigation events.

Minimum Qualifications
Communicates verbally and in writing, to analyze problems, situations and suggest solutions, leadership, follow directions and establish priorities
Skills in physical examination, health assessment, and interviewing and counseling technique
Ability to function independently, prioritize and organize work, solve problems, adapt to change, function as a team member and relate to the public
Knowledge of public health concepts, principles of public health nursing, principles of epidemiology; immunizations; basic nursing science, medical terminology, case management
Must be certified in Basic Life Support (BLS)/CPR
Requires licensure as an RN in Virginia or compact state. If a new graduate, must obtain RN license within 90 days of the date of hire or subject to termination per the regulations governing the practice of nursing. Requires BLS certification. Requires adherence to all safety policies including a review of immunizations, driver safety policies and all other state policies.

Additional study in health promotion/or public health is preferred
BSN preferred
Experience in a public health setting or one related to disease prevention and health promotion
