Previous experience in Wound and Ostomy Care (WOC) preferred.

If no WOC experience, team member must successfully obtain wound and ostomy certifications within 12 months of hire (examples, but not limited to: CWON Certified Wound Ostomy Nurse; CWOCN Certified Wound, Ostomy, Continence Nurse; CWCN Certified Wound Care Nurse; and COCN Certified Ostomy Care Nurse)

The home care RN is a dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a multidisciplinary team.

The Home Health Care RN utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care for a variety of complex patients in collaboration with patients, families, physicians, and the Continuum therapy staff. Continuum’s goal is to educate and empower patients and families and achieve exceptional patient outcomes. Specific skills you will use and develop as a Continuum home health nurse are complex wound care, infusion care, and more!!

The Home Health Care RN is an experienced clinician focused on expanding knowledge and skills. Consistently provides effective direct care to patients in their homes for an assigned caseload of patients; as part of the interdisciplinary team; oversees the care planning and care coordination for a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients their families Physicians and other members of the health care team and in compliance with homecare standards and regulatory requirements. Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process.

Position Compensation Range: $82,992.00 - $110,240.00 Annual

MINIMUM REQUIREMENTS:
  • Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.
  • Experience: 1 year of Relevant Experience
  • Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver’s License required. American Heart Association (AHA) Health Care Provider BLS certification required.


Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
