Responsible for the intake and logistics of facilitating outside hospital requests for patient transfer into the University of Virginia Medical Center. Tasks include: identification and triage of calls to appropriate service lines communication with accepting physicians coordination with UVA’s Medical Transport dispatch coordination with the UVA Bed Center and serving as the primary liaison between referring facilities and UVA physicians nursing and ancillary staff in order to facilitate and safe and efficient patient transfer.

  • As an experienced Registered Nurse, the RN Transfer Coordinator is responsible for the intake and logistics of facilitating outside hospital patient transfer requests for all clinical divisions and acuities. Tasks include: identification and triage of calls to appropriate service lines, communication with accepting physicians, coordination with UVA’s Medical Transport Network dispatch team (MEDCOM), coordination with UVA Bed Center, and serving as the primary liaison between referring facilities, UVA physicians, nursing, and ancillary staff, in order to facilitate safe and efficient patient transfer. Coordinator is responsible for communicating with various internal and external customer involved with patient transfers and medical center operations, and initiate appropriate escalate for services if needed. Coordinator will use clinical experience and knowledge to gather the appropriate clinical information needed to provide patient clinical presentations and updates to UVA physician teams, and will escalate any identified concerns for patient condition changes to the appropriate teams, based on the information provided by the referring facility.


Position Compensation Range: $34.61 - $55.38 Hourly

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Serves as front-line liaison between outside referring facility customers and the UVA Health System for all non-intensive care and acute care level patients Understands and properly uses all communication equipment, programs, and technology Maintains a positive and professional work environment Identifies potential areas for improved operations and care Performs telephonic patient triage and processes requests for assistance with acute patient transfers and/or admissions of patients from medical facilities Demonstrates the knowledge and skills to secure relevant patient care data and identify key patient issues; understanding the relationship between assessments, interventions, and patient responses; skilled in documentation, assessments and cognitive interventions. Provides medical interpretation of diagnoses and specialized patient care needs, recognizing critical and emergent situations, and connecting the referring physician with the appropriate UVA physician/service Provides clinical insights and overall level of care information to physicians Provides health care access through the accurate gathering of demographic, sponsorship or guardian data, as well as insurance, financial and statistical information from a variety of sources Demonstrates the ability to triage and prioritize patient information and communicate an accurate clinical presentation of the patient for accepting physicians Identifies health needs based on assessment and utilization of guidelines. Facilitates appropriate disposition based on assessment, triage guideline parameters and clinical judgment In addition to the above job responsibilities, other duties may be assigned

MINIMUM REQUIREMENTS:
  • Education: Graduate of an accredited nursing program/associates degree in nursing (ADN) required. Bachelors of Science in Nursing Degree is preferred
  • Experience: 3 years of experience as a med/surg, critical care, or ED bedside RN is required; 5+ years of experience at the bedside in these areas is preferred.
  • License/Certification: Practice as a Registered Nurse in the Commonwealth of Virginia is required.
