Job Duties
Hiram W. Davis Medical Center, located in Petersburg, Virginia, is a 94-bed medical facility providing acute medical diagnoses, skilled care, intermediate care, and long term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities. Admissions to HWDMC are generally either permanent placement or special hospitalization. Patients typically come to HWDMC as a special hospitalization for step-down to stabilize, post-surgery rehabilitation, or long-term care.
The Social Worker will provide a full range of professional social work, counseling and case management services in an autonomous medical long-term care facility for the mentally ill, intellectually disabled and geriatric population.
Conducts Social Work Assessments, Social Histories, and Quarterly Reviews.
Develops appropriate care plans based on the individual's needs to include discharge planning.
Completes documentation for these assessments and reviews according within timeframes established by CMS, TJC, and HDMC Policy.
Counsels individuals and families in order to assist them in their adjustment to emotional, physical, mental, and financial hardships.
Maintains regular contact with individuals on caseload, their families/ARs/legal guardians, and CSB Support Coordinators. Communicates concerns to appropriate individual or department and document communication in the Medical Record in a timely manner.
Initiates, plans, and coordinates discharge planning for permanent nursing home individuals with a primary diagnosis of behavioral health and for patients in General Medical/Hospital beds in accordance with CMS regulations, TJC standards, and HDMC Policies.
Collaborates with clinical and administrative staff in scheduling and preparing for recommitment hearing process.
Minimum Qualifications
BSW from an accredited School of Social Work
Must hold a valid Virginia Driver's License in good standing with DMV
Working knowledge of principles and practices of social work services
Additional Considerations
Experience in case management, social work and discharge planning
Experience working with individuals with behavioral health and ID/DD diagnoses
Experience with individual and family counseling pertaining to a nursing home/medical work environment and end of life issues
