RN Case Manager, Blue Ridge Care Connection for Children

The Department of Pediatrics at the University of Virginia’s School of Medicine seeks applicants for an RN Case Manager to support the Blue Ridge Care Connection for Children (BRCCC). The Blue Ridge Care Connection for Children program provides community-based services and funds to support families with children with special healthcare needs. Our professional care coordinators help children, and their families access medical and community-based services targeted to each child's individual needs.

The ideal candidate will utilize advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers, and optimizes patient outcomes throughout all transitions within the continuum of care. More than 190,000 children in Virginia have special healthcare needs - physical disorders that are expected to last for at least 12 months. These children need treatment beyond the norm and may face social, academic, or athletic challenges. Six regional programs operate throughout the Commonwealth. Each program is managed locally, maximizing the program's ability to make efficient and effective use of the area's unique resources.

Job Responsibilities
  • Relationship Based Care - Patients and Families: reflects the influence of the nurse’s relationship with self, colleagues and patient/family on the patient’s experience.
  • Quality Achievement: includes adherence to clinical documentation guidelines, comprehension of outcomes data, engagement in performance improvement activities and commitment to standard work.
  • Innovation: is demonstrated by application of technologies that support patient care, actively seeking to implement evidence-based practice and new knowledge generated by nursing research.
  • Identify problems, goals, and interventions designed to meet outpatient needs, including prioritized goals that consider the patient/caregivers goals, preferences and desired level of involvement in the case management.
  • Provide appropriate interventions which demonstrates knowledge of and sensitivity towards cultural diversity and religious beliefs, developmental, health literacy, and educational background of the population served by Blue Ridge Care Connection for Children.
  • Provide referrals to appropriate community resources: facilitate access and communication when multiple services are involved.
  • Monitor activities to ensure that services are being delivered and meet the needs of the patient, and coordinate services to avoid duplication.
  • Communicate with primary care physicians and members of the comprehensive care team regarding the status of the patient.
  • Maintain accurate patient records and patient confidentiality.
  • Case Management Certification must be obtained within five years of hire and maintained during employment.
  • In addition to the above job responsibilities, other duties may be assigned.


This job description integrates the ANA 2015 Nursing: Scope and Standards of Practice, Third Edition and the ANA Code of Ethics for Nurses with Interpretive Statements (ANA, 2015), and the UVA Nursing Professional Practice Model. ANA Scope and Standards of Practice definition of nursing: Nursing is the protection, promotion, and optimization of health and abilities, prevention of illness and injury, facilitation of healing, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, groups, communities and populations.

Minimum Qualifications

Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.
Experience: One year of relevant experience required
Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.

Physical Demands

Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious diseases.
