Ambulatory Clinical Pharmacist- Endocrinology

The Ambulatory Care Clinical Pharmacist for Endocrinology collaborates to provide medication management, medication access, and patient education in order to optimize clinical outcomes. This position primarily supports a large multidisciplinary team within the UVA Health Endocrinology clinic. Clinical activities include but are not limited to: 1) collaboration with providers for the management of endocrine disease states including, but not limited to, diabetes, osteoporosis, pituitary disorders, thyroid disease and gender health 2) medication counseling and follow up monitoring for endocrinology specialty and non-specialty medications, 3) assistance with medication information questions from the care team, adverse drug events, dose adjustments and management of endocrine-related medications, and 4) optimizing medication access for patients.

Education-related opportunities include: serving as a rotation and/or project preceptor for APPE students and residents (PGY1, PGY2 Ambulatory Care, PGY2 Ambulatory Care), and didactic lectures/in-services for staff and trainees. This individual will promote UVA Specialty Pharmacy’s culture of caring and communication, which fosters patient empowerment through education and support.

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

PREFERRED REQUIREMENTS
• Postgraduate Year Two (PGY2) Pharmacy Residency, or an equivalent amount of relevant work experience
• Credentialed as Board Certified Pharmacotherapy Specialist (BCPS), Ambulatory Care Pharmacist (BCACP), Certified Diabetes Care and Education Specialist (CDCES) and/or Board Certified - Advanced Diabetes Management (BC-ADM) certification.

  • Demonstrates proficiency in Pharmacy distributive services.
  • Supports the educational and professional development of the Pharmacy department.
  • Engages in professional self-development.
  • In addition to the above job responsibilities, other duties may be assigned.
Position Compensation Range: $52.46 - $83.94 Hourly

MINIMUM REQUIREMENTS

Education: Bachelor’s in Pharmacy or Pharm. D Degree required.

Experience: PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas.

Licensure: Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider. BLS certification required
