Job Duties
Direct Patient Care Provides direct care to physically ill patients with diverse emotional problems and developmental disabilities ranging in age from adolescent to geriatric. Personal hygiene duties include: bathing, changing linen and making up beds, washing and combing hair, cleaning and trimming nails, providing oral care and maintaining clean, orderly patient rooms including bedside stands, tables and wardrobes. Routine Treatment/Tasks Performs routine treatments to patients as assigned including, but not limited, skin care, non-sterile warm or cold compresses, enemas, and emptying Foley bags, completing intake and output and colostomy irrigation/care. Assists with patient admission, transfers or discharges (including deaths). Collects and charts specimens and cleans equipment that has been used by the patient. Follows policies and procedures related to clothing inventory, laundry, labeling and maintaining of patient clothing and belongings. Optimizes Patients Functional Performance Encourages patients to achieve their optimum activity level as ordered by the physician including positioning, range of motion exercises, assisting patients with walking, transferring and sitting up in the chair. Performs correct application of splint and braces, according to scheduled time, and monitors for pressure areas. Becomes competent in bedside range of motion and actively participates in restorative care activities following skills enhancements. Patient Observation and Documentation Take vital signs and charts on the graphic sheet and in the patient’s medical record. Observes, records and reports to supervisor changes in-patient’s signs, symptoms or condition. Documents care given, charts TPR's input and output and complete reports according to HWDMC policies and procedures. Restorative Feeding Assists with setting meal trays and encourages patients to eat their prescribed diets, serves between meal nourishment and documents patients to feed self, and sets best dining atmosphere for patients. Patient Transportation & 1:1 Supervision Transports patients within HDMC and to include special hospitalization or outpatient procedures. Provides 1:1 supervision to patients according to policy, within HDMC and to include patients on special hospitalization
Minimum Qualifications
Certified as a nursing assistant with the Virginia State Board of Nursing.
KSA's Required to Perform the Job Successfully: Knowledge of personal care needs and activities of daily living; ability to carry out the responsibilities of the position; ability to read, write and communicate effectively with others; and possess the physical ability to turn, bend and lift non-ambulatory adult patients without restrictions. Must be able to work required overtime and perform CPR. Performance Management (for employees who supervise others) Staff's expectations are clear, well communicated and relate to the goals and objectives of the department or unit; her staff receive frequent, constructive feedback and coaching, including interim evaluations as appropriate. Staff will have the necessary knowledge, skills, and abilities to accomplish goals and are evaluated for competency throughout the evaluation cycle; She ensures requirements of the performance planning and evaluation system are met and evaluations are completed by established deadlines with proper documentation. Staff will ensure performance issues are addressed and documented as they occur in accordance with the Standards of Conduct; Staff will address unsatisfactory attendance and punctuality and documents them as they occur in accordance with the Attendance Policy and the Standards of Conduct. Staff will work to continuously improve safety in all areas and ensure that all tasks are carried out in a safe and efficient manner complying with established safety regulations; balancing quality, cost, and safety to achieve positive results; and ensures staff have completed all mandated training requirements unless documented. Work Ethics and Personal Accountability Staff will demonstrate respect, openness & responsiveness to the needs of others and through consistently helpful behavior & positive attitude, fosters a productive work environment that is free from controversy & hostile interpersonal relationships. Staff will maintain. high standards of professional conduct & accountability in every aspect of job performance and acts discreetly & confidentially in all matters requiring careful handling. She practices and promotes open & effective communication up, down & across the organizational structure and takes responsibility for offering potential solutions when identifying problems in the work setting. Staff will possess an attitude and behavior at work which supports and advances the facility’s vision and mission.
Additional Considerations
Working experience as a nursing assistant within the field of medical-surgical, psychiatric, rehabilitation, geriatrics or intellectually disabled preferred.
