Job Duties

The Social Services Director directs, manages and supervises professional services and staff of the social services department. Evaluate the social and emotional needs of each resident admitted and formulate interventions to meet needs. Review and revise the plan of care as necessary. Refer residents and families to community resources when appropriate. Assure compliance with Local, State, and Federal regulations and guidelines that apply to his/her areas of responsibility.
Provide support to family members/responsible parties/guardians in the overall care and treatment of the residents. Documenting/Charting in Resident files. Develop staff specific approaches and interventions. Pending discharges, work with resident’s families/responsible parties for discharge planning and adjustment. Competencies will be attached with the performance review section.

Minimum Qualifications

• Minimum of a Bachelor’s degree in social work; OR
• Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
• One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals
